I got the chance to work with a really great company at yourpicturebook.com. The company takes a photo of your child, and your child's name and edits it into the cover, as well as every page spread of the book! I've seen a few companies who make personalized photo books for children, but instead of using your entire child, they cut out your childs face and then slam it down on a cartoon childs body.
